1. Scope
These Terms of Use (“Terms”) apply to the use of the visitor registration form (the “Form”) for the Memorial Service for Kazuo Nakamura, jointly hosted by CMIC HOLDINGS Co., Ltd. and Artemis Corporation.

2. Registration
-The Form is provided to support smooth reception and guidance on the day of the event.

-If any issues or omissions are found in the submitted information, we may contact you for confirmation.

-If duplicate registrations by the same individual are identified, the organizers may consolidate or delete such entries at their discretion.

3. Changes and Cancellations
In principle, any changes or cancellations after submitting the Form should be made via the method indicated in the confirmation email sent after registration ((i.e.,the change/cancellation form).

4. Prohibited Activities
The following actions are prohibited when using the Form:
-Submitting false or inaccurate information
-Impersonating another individual
-Interfering with, or attempting to interfere with, the operation of the Form
-Any other actions deemed inappropriate by the organizers

5. Disclaimer
The organizers shall not be liable for any damages resulting from an inability to use the Form due to communication failures, system issues, or other reasons beyond the organizers’ reasonable control (except in cases of willful misconduct or gross negligence by the organizers).

6. Handling of Personal Information
Personal information collected through the Form will be used by CMIC HOLDINGS Co., Ltd. and Artemis Corporation for the following purposes:
-Registration management, reception and guidance on the day of the event, and post-event communications
-Event-related communications, including important notices such as schedule changes
-Responding to inquiries
Personal information will be handled only within the scope necessary to achieve these purposes, in accordance with the “CMIC Group Privacy Policy(https://en.cmicgroup.com/about-cmic/policies/#b2d61b959659d8df9)”, with appropriate security measures in place.
Such personal information may be jointly used by CMIC Group companies and their designated managers, as described in the “ the Matters Published Pursuant to the Act on the Protection of Personal Information(https://www.cmicgroup.com/corporate/privacy-statement)” within the scope of the purposes stated above.
Personal information will not be provided to any third party without the individual’s consent, except as required by law. However, personal information may be entrusted to contractors to the extent necessary for event operations.

7. Photos and Video Recording
Photos and video may be taken at the venue for record-keeping purposes.
Recorded materials will be managed by the organizers, and if published externally, care will be taken to ensure that individuals are not identifiable.

9. Changes to these Terms
